Based on local survey data, the lack of a common referral process and lack of a single-point-of-entry for various services, hinders many families-in-need of receiving eligible services. Research further indicates that when families’ basic needs are not met, they are less likely to seek early childhood services which is apparent in a recent environmental scan of children 0-3 from lower-income families receiving early childhood services in the Rockford area. This ITP™ is seeking service providers to serve as Single-Point-Of-Entry (SPOE) for various services offered in the Rockford area. Identified SPOEs will be listed with key contact information on a referral card to be distributed to agencies, locations, and persons in the community who serve families with children 0-5 years old including case managers, clerks, and others.
